As the First Insured, I acknowledge and confirm the following:
FIRST: I am receiving a life insurance policy, and my agent has clearly explained it to me, including through a video titled “DELIVERY.” By signing this document, I certify that I have reviewed and understood the policy, and I agree that the only benefits provided are those described in the official documentation issued by the insurance company, which I confirm I have received in full. I also understand that surrendering the policy before the stipulated time may incur a financial cost. The delivery video referenced herein explains this in detail.
SECOND: I understand and agree that a life insurance policy is not an investment and is not linked to any form of investment vehicle.
THIRD: I declare that my decision to accept this policy is based solely and exclusively on the benefits described in the “FIRST” paragraph.
As the First Insured, I clearly understand that this is an Indexed Universal Life (IUL) policy, and that IT IS NOT LINKED TO ANY KIND OF INVESTMENT. I also affirm that I HAVE NOT BEEN INDUCED OR MOTIVATED BY ANY PROMISES. I accept this policy solely based on the benefits described in the "Indexed Universal Life Buyer’s Guide" included in the policy package I received.
LEGAL PROTECTION CLAUSES
APPLICABLE LAW AND JURISDICTION:
This contract shall be governed by and interpreted exclusively under the laws of the State of North Carolina, United States of America. Any dispute, controversy, or claim arising from or related to this document or the associated policy shall be resolved exclusively in the state or federal courts located in Mecklenburg County, North Carolina.
ARBITRATION AND WAIVER OF CLASS ACTIONS:
Any such dispute shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association (AAA), and both parties expressly waive the right to participate in any class action lawsuits.

Yo, como Primer Asegurado, reconozco y confirmo lo siguiente:
PRIMERO: Estoy recibiendo una póliza de seguro de vida, la cual me ha sido explicada claramente por mi agente, incluyendo un video titulado “ENTREGA.” Mediante este documento, certifico que he revisado y comprendido la póliza, y estoy de acuerdo en que los únicos beneficios ofrecidos son los descritos en la documentación oficial emitida por la aseguradora, la cual declaro haber recibido en su totalidad. También entiendo que rendirme de la póliza antes del tiempo estipulado puede conllevar un costo financiero. El video de entrega referido explica esto en detalle.
SEGUNDO: Entiendo y acepto que una póliza de seguro de vida no es una inversión ni está vinculada a ningún tipo de instrumento de inversión.
TERCERO: Declaro que mi decisión de aceptar esta póliza se basa exclusivamente en los beneficios descritos en el párrafo “PRIMERO.”
Como Primer Asegurado, entiendo claramente que esta es una póliza de Index Universal Life (IUL), y que NO ESTÁ VINCULADA A NINGÚN TIPO DE INVERSIÓN. Asimismo, afirmo que NO HE SIDO MOTIVADO NI INDUCIDO POR NINGUNA PROMESA. Acepto esta póliza únicamente por los beneficios descritos en la “Guía del Comprador de Index Universal Life” incluida en el paquete de documentos que recibí.
CLÁUSULAS DE PROTECCIÓN LEGAL
LEY APLICABLE Y JURISDICCIÓN:
Este contrato se regirá e interpretará exclusivamente conforme a las leyes del Estado de Carolina del Norte, Estados Unidos de América. Cualquier disputa, controversia o reclamo que surja en relación con este documento o la póliza correspondiente será resuelto exclusivamente en los tribunales estatales o federales ubicados en el Condado de Mecklenburg, Carolina del Norte.
ARBITRAJE Y RENUNCIA A DEMANDAS COLECTIVAS:
Cualquier disputa será resuelta mediante arbitraje vinculante conforme a las reglas de la American Arbitration Association (AAA), renunciando expresamente ambas partes al derecho de participar en demandas colectivas.
